usb: renesas_usbhs: fixup interrupt status clear method

When interrupt happened, renesas_usbhs driver gets irq status
by usbhs_status_get_each_irq(), and cleared all status by using 0.
But, this method is incorrect,
since extra interrupt might occur between them.
This patch cleared corresponding bits only

usb: renesas_usbhs: fixup __usbhs_for_each_pipe 1st pos

__usbhs_for_each_pipe() is the macro which moves around each pipe,
but it has a bug which didn't care about 1st pipe's position.
Because of this bug, it moves around
pipe0, pipe2, pipe3 ... even though it requested pipe1, pipe2, pipe3...
This patch modifies it.

Revert "drm/i915: Try harder to complete DP training pattern 1"

This reverts commit 2477367083b3eaa97f87993ab26627a02f350551.

If (for whatever reason) the DP sink device never asks for the maximal
voltage level, we never don't hit the check that should bail us out
after 5 retries of the same voltage. Which leads to an endless loop in
the DP link training code, which hangs the driver.

Now some more DP link training experiments on eDP panels seem to
indicate that our training algorithm isn't robust enough anyway and
needs more work. Hence for 3.7-fixes, let's just revert the regressing
commit instead of trying to apply more duct-tape.

DRM/i915: Restore sdvo_flags after dtd->mode->dtd Roundrtrip.

For TV and LVDS encoders intel_sdvo_set_input_timings_for_mode()
is called to pass a mode to the sdvo chip and retrieve a dtd
containing information needed to calculate the adjusted_mode which
is done by intel_sdvo_get_dtd_from_mode().
To set this adjusted_mode as input mode for the sdvo chip, a dtd is
recalculated using intel_sdvo_get_mode_from_dtd(). During this round
trip the sdvo_flags contained in the dtd obtained from the hardware
are lost.
Since these flags cannot be ignored in all cases this patch preserves
and restores them.

This regression has been introduced in

NCR machines with LVDS panels using Intel chipsets need to have the
QUIRK_INVERT_BRIGHTNESS bit set.
Unfortunately NCR doesn't set a meaningful subvendor/subdevice ID,
therefore we add a DMI dependent quirk list.

DRM/i915: Don't delete DPLL Multiplier during DAC init.

The DPLL multipiler is set up in intel_display.c:i9xx_update_pll()
called from i9xx_crtc_mode_set().
There the DPLL multiplier is adjusted so that the SDVO gets a sufficient
bus clock.
When cloning a CRTC between an SDVO driven encoder and the standard
DAC the DAC setup code reseted the multiplier value to 1 thus undoing
the correct setup. There is no need to touch the multiplier in the DAC
setup code: the correct value (i.e. 1 in case no SDVO encoder is used)
is set by i9xx_update_pll() already.
A comment at the code suggested that this code is a left over from the
days when there was no setup for clone modes.

With the introduction of fan management modes, fan may not be drivable.
We should allow reclocking nonetheless.

This return was stupid to begin with since it may have left the card
in an intermediate state (clocks corresponding to a perflvl and voltage
corresponding to another one). The reclocking code will need to be
rewritten in a near-future in order to provide a better error handling.

Boot parameter tags with handlers are ignored like this:

[    0.000000] Ignoring tag 0x00001003
[    0.000000] Ignoring tag 0x00001001
[    0.000000] Ignoring tag 0x00001004

because neither tagtable entries nor tag handlers appear in the vmlinux.

Fix tagtable definition attributes so that tag entries are not dropped.

Fix end of memory bank calculation in parse_tag_mem: it is intended to
round down to page size, but instead did something strange leading to
hang right after boot.

xtensa: copy_thread with CLONE_VM must not copy live parent AR windows

When doing a fork (new VM), the new task has a mirror image of the
parent's stack, so keeps the same live register windows etc.
However when doing a clone with CLONE_VM, keeping the same VM
(eg. when creating a new thread), the child starts afresh on a new
stack -- it cannot share any part of the parent stack.  It
especially cannot have the same live AR windows as the parent,
otherwise it will overwrite the parent stack on overflow, likely
causing corruption.  (and so it did...)

Effectively, the register windows need to be spilled.
Turns out it's much easier to simply not copy parent register
windows when CLONE_VM is set.

ISS serial console prints garbage instead of symbols printed via
rs_put_char. gcc optimizes away putting prined symbol into memory buffer
because there's no evidence that the buffer is used afterwards.

Make rs_put_char and rs_write use simc_write that has explicit wmb.

Commit 023614183768 ("thermal: add generic cpufreq cooling
implementation") requires cpufreq_frequency_get_table(), but that
function is only defined for CONFIG_CPU_FREQ_TABLE resulting in the
following build error:

  drivers/thermal/cpu_cooling.c:259: undefined reference to `cpufreq_frequency_get_table'
  drivers/built-in.o: In function `get_cpu_frequency':

Fix it by selecting CONFIG_CPU_FREQ_TABLE for such a configuration.

It turns out CONFIG_EXYNOS_THERMAL also needs CONFIG_CPU_FREQ_TABLE, so
select it there as well.

drm/radeon: Don't destroy I2C Bus Rec in radeon_ext_tmds_enc_destroy().

radeon_i2c_fini() walks thru the list of I2C bus recs rdev->i2c_bus[]
to destroy each of them.
radeon_ext_tmds_enc_destroy() however also has code to destroy it's
associated I2C bus rec which has been obtained by radeon_i2c_lookup()
and is therefore also in the i2c_bus[] list.
This causes a double free resulting in a kernel panic when unloading
the radeon driver.
Removing destroy code from radeon_ext_tmds_enc_destroy() fixes this
problem.

Based on Dmitries work, but splitting the code into page
directory and page table handling makes it far more
readable and (hopefully) more reliable.

Allocations of page tables are made from the SA on demand,
that should still work fine since all page tables are of
the same size.

Also using the fact that allocations from the SA are mostly
continuously (except for end of buffer wraps and under very
high memory pressure) to group updates send to the chipset
specific code into larger chunks.

ext4: fix undefined bit shift result in ext4_fill_flex_info

The result of the bit shift expression in
'1 << sbi->s_log_groups_per_flex' can be undefined in the case that
s_log_groups_per_flex is 31 because the result of the shift is bigger
than INT_MAX. In reality this probably should not cause much problems
since we'll end up with INT_MIN which will then be converted into
'unsigned int' type, but nevertheless according to the ISO C99 the
result is actually undefined.

Fix this by changing the left operand to 'unsigned int' type.

Note that the commit d50f2ab6f050311dbf7b8f5501b25f0bf64a439b already
tried to fix the undefined behaviour, but this was missed.

Thanks to Laszlo Ersek for pointing this out and suggesting the fix.

The analysis of this is as follows.  In init/main.c, we issue:

kernel_thread(kernel_init, NULL, CLONE_FS | CLONE_SIGHAND);

This creates a new thread, which falls through to the ret_from_fork
assembly, with r4 set NULL and r5 set to kernel_init.  You can see
this in your oops dump register set - r5 is 0xc0344555, which is the
address of kernel_init plus 1 which marks the function as Thumb code.

Now, let's look at this code a little closer - this is what the
disassembly looks like:

This code was introduced in 9fff2fa0db911 (arm: switch to saner
kernel_execve() semantics).  I have marked one instruction, and it's
the significant one - I'll come back to that later.

Eventually, having had a successful call to kernel_execve(), kernel_init()
returns zero.

In returning, it uses the value in 'lr' which was set by the instruction
I marked above.  Unfortunately, this causes lr to contain 0xc000d18e -
an even address.  This switches the ISA to ARM on return but with a non
word aligned PC value.

So, what do we end up executing?  Well, not the instructions above - yes
the opcodes, but they don't mean the same thing in ARM mode.  In ARM mode,
it looks like this instead:

I have included more above, because it's relevant.  The PSR flags which
we can see in the oops dump are nZCv, so Z and C are set.

All the above ARM instructions are not executed, except for two.
c000d1a0, which has no writeback, and writes below the current stack
pointer (and that data is lost when we take the next exception.) The
other instruction which is executed is c000d1ac, which takes us to...
0xc03493dc.  However, remember that bit 1 of the PC got set.  So that
makes the PC value 0xc03493de.

And that value is the value we find in the oops dump for PC.  What is
the instruction here when interpreted in ARM mode?

and there we have our undefined instruction (remember that the 'never'
condition code, 0xf, has been deprecated and is now always executed as
it is now being used for additional instructions.)

This path also nicely explains the state of the stack we see in the oops
dump too.

The above is a consistent and sane story for how we got to the oops
dump, which all stems from the instruction at 0xc000d18a being wrong.

There is nothing to prevent another thread from dereferencing ds->ds_clp
during or after the call to nfs4_ds_disconnect(), and Oopsing due to the
resulting NULL pointer.

Instead, we should just rely on filelayout_mark_devid_invalid() to keep
us out of trouble by avoiding that deviceid.

If a USB transfer has already been started, meaning
we have already issued StartTransfer command to that
particular endpoint, DWC3_EP_BUSY flag has also
already been set.

When we try to cancel this transfer which is already
in controller's cache, we will not receive XferComplete
event and we must clear DWC3_EP_BUSY in order to allow
subsequent requests to be properly started.

The best place to clear that flag is right after issuing
DWC3_DEPCMD_ENDTRANSFER.

The compilation of the pinctrl driver failed on the legacy
Nomadik NHK8815 platform because it was not providing the PRCMU
interfaces needed to support the extended alternate functions
used by the ux500 series.

Solve this by providing some stubs for the legacy platform, to
avoid too much #ifdefs in the code per se. Theoretically this
actually allows the Nomadik and Ux500 to have a single kernel
image with support for the PRCM registers on the Ux500 (though
they have incompatible archs, but the spirit is there).

The prima2 platform advertises needing no mach/gpio.h header file,
but its pinctrl driver now has a sirfsoc_gpio_set_pull function
that uses constants defined in arch/arm/mach-prima2/include/mach/gpio.h,
which fails to build.

Fortunately, the sirfsoc_gpio_set_pull is not used anywhere in the
kernel, so we can safely remove it. Any out of tree drivers using
it will have to be converted to use proper pinctrl functions to
do the same.

Without this patch, building prima2_defconfig results in:

pinctrl: samsung: use __devinit section for init code

The samsung pinctrl driver has a probe function that is
__devinit and that calls a lot of other functions that are
marked __init, which kbuild complains about.

Marking everything __devinit means that the code does not
discarded when CONFIG_HOTPLUG is set, which is a little
more wasteful, but also more consistent

Without this patch, building exynos_defconfig results in:

WARNING: drivers/pinctrl/built-in.o(.devinit.text+0x124): Section mismatch in reference from the function samsung_pinctrl_probe() to the function .init.text:samsung_gpiolib_register()
The function __devinit samsung_pinctrl_probe() references
a function __init samsung_gpiolib_register().
If samsung_gpiolib_register is only used by samsung_pinctrl_probe then
annotate samsung_gpiolib_register with a matching annotation.

TjMax for the CE4100 series of Atom CPUs was previously reported to be
110 degrees C.

cpuinfo logs on the web show existing CPU types CE4110, CE4150, and CE4170,
reported as "model name : Intel(R) Atom(TM) CPU CE41{1|5|7}0 @ 1.{2|6}0GHz"
with model 28 (0x1c) and stepping 10 (0x0a). Add the three known variants
to the tjmax table.

The hostprogs need access to the CONFIG_* symbols found in
include/generated/autoconf.h.  But commit abbf1590de22 ("UAPI: Partition
the header include path sets and add uapi/ header directories") replaced
$(LINUXINCLUDE) with $(USERINCLUDE) which doesn't contain the necessary
include paths.

This has the undesirable effect of breaking the EFI boot stub because
the #ifdef CONFIG_EFI_STUB code in arch/x86/boot/tools/build.c is
never compiled.

It should also be noted that because $(USERINCLUDE) isn't exported by
the top-level Makefile it's actually empty in arch/x86/boot/Makefile.

Hi Greg,

Here's four fixes for the USB 3.0 Link Power Management code.  They
work around some USB 3.0 devices that don't support LPM, like the
Western Digital My Passport hard drive.  They also fix an LPM disable
ref count bug that would cause LPM to remain disabled for a device
after a failed probe.

Please queue for 3.7 after the merge window closes.  Several patches are
marked for stable.

 (1) Remove kernel-only stuff in linux/ppp-comp.h from the UAPI.  I checked
     this with Paul Mackerras before I created the patch and he suggested some
     extra bits to unexport.

 (2) Remove linux/blk_types.h entirely from the UAPI as none of it is userspace
     applicable, and remove from the UAPI that part of linux/fs.h that was the
     reason for linux/blk_types.h being exported in the first place.  I
     discussed this with Jens Axboe before creating the patch.

 (3) The big patch of the series to disintegrate include/linux/*.h as a unit.
     This could be split up, though there would be collisions in moving stuff
     between the two Kbuild files when the parts are merged as that file is
     sorted alphabetically rather than being grouped by subsystem.

     Of this set of headers, 17 files have changed in the UAPI exported region
     since the 4th and only 8 since the 9th so there isn't much change in this
     area - as one might expect.

     It should be pretty obvious and straightforward if it does come to fixing
     up: stuff in __KERNEL__ guards stays where it is and stuff outside moves
     to the same file in the include/uapi/linux/ directory.

     If a new file appears then things get a bit more complicated as the
     "headers +=" line has to move to include/uapi/linux/Kbuild.  Only one new
     file has appeared since the 9th and I judge this type of event relatively
     unlikely.

 (4) A patch to disintegrate include/linux/byteorder/*.h as a unit.

  This is a pet peeve of mine.  Any time there's a long list of items
  (header file inclusions, kconfig entries, array initalisers, etc) and
  someone wants to add a new item, they *always* go and stick it at the
  end of the list.

  Guys, don't do this.  Either put the new item into a randomly-chosen
  position or, probably better, alphanumerically sort the list.
