This patch updates the DMA Coalescing feature parameters to account for
larger MTUs. Previously, sufficient space may not have been allocated in
the receive buffer, causing packet drop.

e1000e: hitting BUG_ON() from napi_enable

Based on a patch from Mike McElroy created against the out-of-tree e1000e
driver:

Hitting the BUG_ON in napi_enable(). Code inspection shows that this can
only be triggered by calling napi_enable() twice without an intervening
napi_disable().

I saw the following sequence of events in the stack trace:

1) We simulated a cable pull using an Extreme switch.
2) e1000_tx_timeout() was entered.
3) e1000_reset_task() was called. Saw the message from e_err() in the
console log.
4) e1000_reinit_locked was called. This function calls e1000_down() and
e1000_up(). These functions call napi_disable() and napi_enable()
respectively.
5) Then on another thread, a monitor task saw carrier was down and executed
'ip set link down' and 'ip set link up' commands.
6) Saw the '_E1000_RESETTING'warning fron the e1000_close function.
7) Either the e1000_open() executed between the e1000_down() and e1000_up()
calls in step 4 or the e1000_open() call executed after the e1000_up()
call.  In either case, napi_enable() is called twice which triggers the
BUG_ON.

Descriptors may not be write-back while checking TX hang with flag
FLAG2_DMA_BURST on.
So when we detect hang, we just flush the descriptor and detect
again for once.

-v2 change 1 to true and 0 to false and remove extra ()

x86: Fix boot failures on older AMD CPU's

People with old AMD chips are getting hung boots, because commit
bcb80e53877c ("x86, microcode, AMD: Add microcode revision to
/proc/cpuinfo") moved the microcode detection too early into
"early_init_amd()".

At that point we are *so* early in the booth that the exception tables
haven't even been set up yet, so the whole

rdmsr_safe(MSR_AMD64_PATCH_LEVEL, &c->microcode, &dummy);

doesn't actually work: if the rdmsr does a GP fault (due to non-existant
MSR register on older CPU's), we can't fix it up yet, and the boot fails.

Fix it by simply moving the code to a slightly later point in the boot
(init_amd() instead of early_init_amd()), since the kernel itself
doesn't even really care about the microcode patchlevel at this point
(or really ever: it's made available to user space in /proc/cpuinfo, and
updated if you do a microcode load).

If our TCP_PAGE(sk) is not shared (page_count() == 1), we can set page
offset to 0.

This permits better filling of the pages on small to medium tcp writes.

"tbench 16" results on my dev server (2x4x2 machine) :

Before : 3072 MB/s
After  : 3146 MB/s  (2.4 % gain)

tcp: take care of misalignments

We discovered that TCP stack could retransmit misaligned skbs if a
malicious peer acknowledged sub MSS frame. This currently can happen
only if output interface is non SG enabled : If SG is enabled, tcp
builds headless skbs (all payload is included in fragments), so the tcp
trimming process only removes parts of skb fragments, header stay
aligned.

Some arches cant handle misalignments, so force a head reallocation and
shrink headroom to MAX_TCP_HEADER.

Dont care about misaligments on x86 and PPC (or other arches setting
NET_IP_ALIGN to 0)

This patch introduces __pskb_copy() which can specify the headroom of
new head, and pskb_copy() becomes a wrapper on top of __pskb_copy()

xen/pm_idle: Make pm_idle be default_idle under Xen.

The idea behind commit d91ee5863b71 ("cpuidle: replace xen access to x86
pm_idle and default_idle") was to have one call - disable_cpuidle()
which would make pm_idle not be molested by other code.  It disallows
cpuidle_idle_call to be set to pm_idle (which is excellent).

But in the select_idle_routine() and idle_setup(), the pm_idle can still
be set to either: amd_e400_idle, mwait_idle or default_idle.  This
depends on some CPU flags (MWAIT) and in AMD case on the type of CPU.

In case of mwait_idle we can hit some instances where the hypervisor
(Amazon EC2 specifically) sets the MWAIT and we get:

In the case of amd_e400_idle we don't get so spectacular crashes, but we
do end up making an MSR which is trapped in the hypervisor, and then
follow it up with a yield hypercall.  Meaning we end up going to
hypervisor twice instead of just once.

The previous behavior before v3.0 was that pm_idle was set to
default_idle regardless of select_idle_routine/idle_setup.

We want to do that, but only for one specific case: Xen.  This patch
does that.

net: Add Open vSwitch kernel components.

Open vSwitch is a multilayer Ethernet switch targeted at virtualized
environments.  In addition to supporting a variety of features
expected in a traditional hardware switch, it enables fine-grained
programmatic extension and flow-based control of the network.
This control is useful in a wide variety of applications but is
particularly important in multi-server virtualization deployments,
which are often characterized by highly dynamic endpoints and the need
to maintain logical abstractions for multiple tenants.

The Open vSwitch datapath provides an in-kernel fast path for packet
forwarding.  It is complemented by a userspace daemon, ovs-vswitchd,
which is able to accept configuration from a variety of sources and
translate it into packet processing rules.

While parsing through IPv6 extension headers, fragment headers are
skipped making them invisible to the caller.  This reports the
fragment offset of the last header in order to make it possible to
determine whether the packet is fragmented and, if so whether it is
a first or last fragment.

The current default watchdog timer is enabled, but we are seeing issues on
legacy devices. So change the default setting of watchdog timer to per
device based. But user still can use the "wd_disable" module parameter
to overwrite the system setting

The ht40 setting should not change after association unless channel switch

This fix a problem we are seeing which cause uCode assert because driver
sending invalid information and make uCode confuse

iwlagn: fix HW crypto for TX-only keys

Group keys in IBSS or AP mode are not programmed
into the device since we give the key to it with
every TX packet. However, we do need mac80211 to
create the MMIC & PN in all cases. Move the code
around to set the key flags all the time. We set
them even when the key is removed again but that
is obviously harmless.

If a device is shutdown, then there might be a pending interrupt,
which will be processed after we reenable interrupts, which causes the
original handler to be run. If the old handler is the (broadcast)
periodic handler the shutdown state might hang the kernel completely.

Bluetooth: Add dev_flags to struct hci_dev

This patch adds the dev_flags field to struct hci_dev. This new
flags variable should be used to define flags related to BR/EDR
and/or LE controller itself. It should be used to define flags
which represents states from the controller. The dev_flags is
cleared in case the controller sends a Reset Command Complete
Event to the host.

Also, this patch adds the HCI_LE_SCAN flag which was created to
track if the controller is performing LE scan or not. The flag
is set/cleared when the controller starts/stops scanning.

This is an initial effort to stop using hdev->flags to define
internal flags since it is exported to userspace by an ioctl.

In irq_wait_for_interrupt(), the should_stop member is verified before
setting the task's state to TASK_INTERRUPTIBLE and calling schedule().
In case kthread_stop sets should_stop and wakes up the process after
should_stop is checked by the irq thread but before the task's state
is changed, the irq thread might never exit:

There are two issues in vmw_kms_update_layout_ioctl().  First, the
for loop forgets to index rects and only checks the first element.
Second, there is a potential integer overflow if userspace passes
in a large arg->num_outputs.  The call to kzalloc() would allocate
a small buffer, leading to out-of-bounds read.

Disabling the CRTC by setting its framebuffer to NULL, as used by
drm_framebuffer_cleanup(), was failing to pass the current framebuffer
to the crtc_func->disable callback. This is because of the dance within
drm_crtc_helper_set_config to pass the new_fb (NULL in this case) to the
drm_crtc_helper_set_mode with the currently attached fb as a parameter.
drm_crtc_helper_set_mode treats this as a no-op and the encoder is still
enabled. And so the current fb is forgotten before the call to
drm_helper_disable_unused_functions.

This patch treats disabling the CRTC as a simple special case rather
than adding further complexity into the configuration logic.

This fixes a pin-leak of the fb bo on Xserver close.

Now RED is classful, we must check q->qdisc->q.qlen, and if queue is empty,
we start an idle period, not end it.

ocfs2: avoid unaligned access to dqc_bitmap

The dqc_bitmap field of struct ocfs2_local_disk_chunk is 32-bit aligned,
but not 64-bit aligned.  The dqc_bitmap is accessed by ocfs2_set_bit(),
ocfs2_clear_bit(), ocfs2_test_bit(), or ocfs2_find_next_zero_bit().  These
are wrapper macros for ext2_*_bit() which need to take an unsigned long
aligned address (though some architectures are able to handle unaligned
address correctly)

So some 64bit architectures may not be able to access the dqc_bitmap
correctly.

This avoids such unaligned access by using another wrapper functions for
ext2_*_bit().  The code is taken from fs/ext4/mballoc.c which also need to
handle unaligned bitmap access.

Commit 1386be55e32a3c5d8ef4a2b243c530a7b664c02c ("dccp: fix
auto-loading of dccp(_probe)") fixed a bug but created a new
compiler warning:

net/dccp/probe.c: In function ‘dccpprobe_init’:
net/dccp/probe.c:166:2: warning: the omitted middle operand in ?: will always be ‘true’, suggest explicit middle operand [-Wparentheses]

try_then_request_module() is built for situations where the
"existence" test is some lookup function that returns a non-NULL
object on success, and with a reference count of some kind held.

Here we're looking for a success return of zero from the jprobe
registry.

Instead of fighting the way try_then_request_module() works, simply
open code what we want to happen in a local helper function.

If we take the "try_again" goto, due to a checksum error,
the 'len' has already been truncated.  So we won't compute

bridge: master device stuck in no-carrier state forever when in user-stp mode

When in user-stp mode, bridge master do not follow state of its slaves, so
after the following sequence of events it can stuck forever in no-carrier
state:
1) turn stp off
2) put all slaves down - master device will follow their state and also go in
no-carrier state
3) turn stp on with bridge-stp script returning 0 (go to the user-stp mode)
Now bridge master won't follow slaves' state and will never reach running
state.

This patch solves the problem by making user-stp and kernel-stp behavior
similar regarding master following slaves' states.

This reverts commit df711fc9962b9491af2b92bd0d21ecbfefe4e5fa.

The commit added a reset-on-resume quirk because the NEC chipset stopped
responding to commands about 30 minutes after a system resume from
suspend.  We thought it was a chipset issue, but it turns out that the
xHCI driver was zeroing out the link TRB after a successful context
restore during resume.  The host controller would fall off the command
ring sometime later, causing it to not respond to new commands.

The link TRB issue has been fixed with commit
158886cd2cf4599e04f9b7e10cb767f5f39b14f1 "xHCI: fix bug in
xhci_clear_command_ring()", so revert the reset-on-resume quirk, as it's
not necessary.

Commit df711fc9962b9491af2b92bd0d21ecbfefe4e5fa was marked for stable
trees back to 2.6.37, but according to my mail, it has not made it into
Linus' tree or the stable trees yet.

xHCI: fix bug in xhci_clear_command_ring()

When system enters suspend, xHCI driver clears command ring by writing zero
to all the TRBs. However, this also writes zero to the Link TRB, and the ring
is mangled. This may cause driver accesses wrong memory address and the
result is unpredicted.

When clear the command ring, keep the last Link TRB intact, only clear its
cycle bit. This should fix the "command ring full" issue reported by Oliver
Neukum.

This should be backported to stable kernels as old as 2.6.37, since the
commit 89821320 "xhci: Fix command ring replay after resume" is merged.

gcc compiler is smart enough to use a single load/store if we
memcpy(dptr, sptr, 8) on x86_64, regardless of
CONFIG_CC_OPTIMIZE_FOR_SIZE

In IP header, daddr immediately follows saddr, this wont change in the
future. We only need to make sure our flowi4 (saddr,daddr) fields wont
break the rule.

This patch adds support for legacy Bosch CC770 and Intel AN82527 CAN
controllers on the ISA or PC-104 bus. The I/O port or memory address
and the IRQ number must be specified via module parameters:

  insmod cc770_isa.ko port=0x310,0x380 irq=7,11

for ISA devices using I/O ports or:

  insmod cc770_isa.ko mem=0xd1000,0xd1000 irq=7,11

for memory mapped ISA devices.

Indirect access via address and data port is supported as well:

  insmod cc770_isa.ko port=0x310,0x380 indirect=1 irq=7,11

Furthermore, the following mode parameter can be defined:

  clk: External oscillator clock frequency (default=16000000 [16 MHz])
  cir: CPU interface register (default=0x40 [DSC])
  bcr: Bus configuration register (default=0x40 [CBY])
  cor: Clockout register (default=0x00)

Note: for clk, cir, bcr and cor, the first argument re-defines the
default for all other devices, e.g.:

  insmod cc770_isa.ko mem=0xd1000,0xd1000 irq=7,11 clk=24000000

is equivalent to

  insmod cc770_isa.ko mem=0xd1000,0xd1000 irq=7,11 clk=24000000,24000000

and the write to it is completely thrown away."

Btrfs: fix meta data raid-repair merge problem

Commit 4a54c8c16 introduced raid-repair, killing the individual
readpage_io_failed_hook entries from inode.c and disk-io.c. Commit
4bb31e92 introduced new readahead code, adding a readpage_io_failed_hook to
disk-io.c.

The raid-repair commit had logic to disable raid-repair, if
readpage_io_failed_hook is set. Thus, the readahead commit effectively
disabled raid-repair for meta data.

This commit changes the logic to always attempt raid-repair when needed and
call the readpage_io_failed_hook in case raid-repair fails. This is much
more straight forward and should have been like that from the beginning.

During reconfig we can get the BSS_CHANGED_AP_PROBE_RESP indication
even if a probe-resp has not been set in the first place. Therefore
ignore the error when not getting a probe-resp from mac80211. Resort to
the legacy probe-resp in this case.

Also take this opportunity to add a vif argument to the set_probe_resp
function.

wl12xx: init CMD_TEMPL_KLV to sizeof(ieee80211_qos_hdr)

The keep alive template should have a max size of
sizeof(struct ieee80211_qos_hdr).

Additionally, Remove the redundant wl12xx_qos_null_data_template
struct.

The wl12xx driver supports probe-response offloading, and the  WPS, WPS2
and P2P special cases as well.

wl12xx: configure probe-resp template according to notification

When operating in AP-mode, replace our probe-response template when a
notification is recieved from mac80211. We preserve the "legacy" way of
configuring a probe-response according to beacon for IBSS mode and for
versions of hostapd that do not support this feature.

wl12xx: Check buffer bound when processing nvs data

An nvs with malformed contents could cause the processing of the
calibration data to read beyond the end of the buffer. Prevent this
from happening by adding bound checking.
